Key Responsibilities\n

• Conduct thorough clinical assessments to understand the unique mental health needs of patients in hospice care.

• Develop, implement, and evaluate individualized treatment plans incorporating evidence-based therapies for each patient.

• Utilize various therapeutic modalities such as cognitive-behavioral therapy, person-centered therapy, and solution-focused therapy to support patients and families effectively.

• Document patient progress and maintain accurate and timely clinical records within our electronic health record (EHR) system.

• Work collaboratively with a multidisciplinary care team, including physicians, nurses, and other mental health professionals, to coordinate patient care.

• Facilitate educational sessions for families regarding grief, coping mechanisms, and resources available for support.

• Stay informed on best practices and emerging research in mental health, particularly in hospice and palliative care settings.

Patient Population / Client Focus\n

The LCSW will primarily work with terminally ill patients and their families, providing critical support in managing complex emotional and psychological challenges during end-of-life transitions. Qualifications\n

Compensation and Benefits\n
