Impactful Case Manager — Elder & Disability Services (WI)

Humana

Humana is seeking a compassionate and organized Case Manager to serve Wisconsin Family Care members in Weston, WI. You will assess needs, coordinate with a Field Care Nurse, and develop a comprehensive MCP, conducting regular in-person and phone follow-ups.

The role includes travel up to 40% and daytime hours, with TB screening and mileage reimbursement provided. Applicants should have a Bachelor's degree in human services or related field and relevant experience; valid driver's license and
