Field Case Manager, Family Care Partnership

Humana is seeking a Case Manager to join the GSR3 team in the Family Care Partnership program across Milwaukee, Racine, Kenosha, and surrounding counties. The role provides assessment, care planning, and ongoing care coordination for adults with disabilities and frail elderly individuals.
Responsibilities include psychosocial assessments, coordinating with resources, monitoring progress, and advocating for member needs.
