RN Care Manager - Care Coordination

Role: RN Care Manager Responsibilities: Facilitate payor authorization, manage patient care across the continuum, collaborate with multidisciplinary teams, ensure documentation accuracy, resolve system and discharge barriers, and monitor patient progress to deliver high-quality, efficient, and cost-effective care. Qualifications: Current IL nursing license, Bachelor's degree (preferably from an NLN-accredited school), clinical certification (beneficial), and a minimum of 2 years clinical nursing experience. Experience in Utilization Management, Managed Care, or chronic disease management, along with EMR familiarity and knowledge of InterQual or MCG criteria, is preferred. Skills: Strong communication, care coordination, problem-solving, and documentation skills.
