As a Licensed Professional Counselor, you will communicate regularly with an integrated care team comprising physicians, nurses, and other mental health professionals. Key Responsibilities\n

• Conduct thorough patient intakes, focusing on diagnostic and functional evaluations to inform treatment plans.

• Deliver evidence-based therapeutic interventions tailored to individual client needs, ensuring compliance with Agency and Federally Qualified Health Center (FQHC) standards.

• Monitor patient progress through regular follow-up evaluations, adjusting treatment plans as necessary based on outcomes and effectiveness.

• Collaborate with primary care providers to recognize which patients require specialized mental health services and facilitate timely referrals.

• Provide immediate crisis intervention as needed, ensuring that patients in acute distress receive focused mental health support.

• Document all patient interactions, diagnostic information, and treatment plans meticulously in the electronic health record system.

• Develop relapse prevention strategies alongside patients and promote self-management skills for sustained recovery.

• Participate in team meetings and case reviews to contribute insights regarding patient care and collaborative treatment planning.

• Engage in continuous professional development by consulting with clinic management and team members on behavioral health services and opportunities for program enhancement.

You will work with a diverse clientele, including individuals facing a range of psychological and emotional challenges. Our patients often present with conditions such as anxiety, depression, trauma, and substance use disorders. Qualifications\n

• A Master's Degree in Clinical Counseling or a closely related field is required.

Compensation and Benefits\n
