Key Responsibilities

In your role as an LCSW, you will engage in a variety of responsibilities designed to provide exceptional care, including:

• Conducting comprehensive clinical assessments to identify client needs and develop individualized treatment plans that leverage evidence-based methodologies.
• Implementing therapeutic interventions such as cognitive-behavioral therapy (CBT) and dialectical behavior therapy (DBT) tailored to client’s unique situations and challenges.
• Maintaining accurate mental health records in compliance with HIPAA and other regulatory standards, ensuring confidentiality and professionalism.
• Collaborating closely with a multi-disciplinary treatment team to provide integrated and coordinated care across all aspects of the client’s mental health journey.
• Participating in regular case consultations and peer review meetings to enhance care quality, share insights, and promote best practices among team members.
• Providing education and resources to patients and their families about treatment options, mental health conditions, and community support services to empower informed decision-making.
• Continuing professional development by staying current with the latest research and innovations in the mental health field to adapt and improve your practice.

Patient Population / Client Focus

Your primary focus will be on individuals receiving hospice care, including patients facing terminal illnesses and their families. As an LCSW, you will address the psychological impact of end-of-life issues, assist clients in coping with grief and loss, and help families navigate the emotional complexities of care decisions. Your role will be critical in easing anxiety and fostering peace, enhancing the quality of life for both patients and their loved ones.

Qualifications

• A Master’s degree in Social Work from an accredited institution, reflecting your dedication to the profession.
• A valid and active state license as a Licensed Clinical Social Worker (LCSW), affirming your ability to practice.
• At least three years of clinical experience in a social work environment, preferably with experience in hospice or palliative care settings.
