External Client Care Coordinator

We are currently seeking a motivated, organized, and relationship-driven External Client Care Coordinator to support client partnerships, coordinate services, and ensure outstanding care delivery across our service areas.

Location:Lebanon/Manheim,Williamstown, Pine Grove, Myerstown, Manheim, Mechanicsburg, Lebanon, Palmyra, Jonestown

What You'll Do

• Build and maintain strong relationships with clients, families, referral partners, and community organizations
  
  
• Serve as a trusted liaison between clients and internal care teams
  
  
• Conduct client consultations and assist with developing personalized service plans
  
  
• Monitor client satisfaction and proactively address concerns or changing needs
  
  
• Coordinate service requests, staffing needs, and ongoing care adjustments
  
  
• Foster partnerships with healthcare professionals, social workers, and community resources
  
  
• Support client retention through consistent communication and quality follow-up
  
  
• Assist with growth initiatives, outreach activities, and community networking opportunities
  
  
• Ensure compliance with company standards and documentation requirements
  
  
• Collaborate with multiple departments to deliver seamless client experiences
