Trauma Patient Navigator
Our Trauma Patient Navigators serve as trusted advocates, community liaisons, and role models for individuals and families impacted by community violence. By building meaningful relationships based on trust, our navigators connect patients and community members with reliable information, appropriate resources, emotional support, and violence prevention services. As a core member of the Social Impact/Project HUDSON team, the Trauma Patient Navigator works collaboratively with hospital and community partners to address barriers to care, promote safety, and support individuals in achieving their goals. We recognize the critical role our navigators play in preventing future violence and improving the health and well-being of the Jersey City community.
A Typical Day:
As a Trauma Patient Navigator, a typical day might include the following:
- Building and maintaining trusting relationships with victims of violence, individuals at risk of violence, and individuals with a history of violence to support violence prevention and intervention.
- Conducting program outreach and developing a caseload of clients for enrollment in the community violence prevention program.
- Completing program intake assessments, obtaining consents, and accurately documenting intake information and outcomes in the client registry.
- Following up with treat-and-release patients by phone to assess needs, provide support, and connect them with appropriate services.
- Assessing clients' community-based needs, identifying barriers to care, and developing individualized service plans based on their goals and circumstances.
- Connecting individuals and families with healthcare, mental health services, social services, and other community-based resources.
- Providing peer support, advocacy, and community-based violence prevention and intervention services throughout Jersey City and Hudson County.
- Collaborating with JCMC programs, community organizations, and the Social Impact/Trauma team to coordinate services and strengthen community partnerships.
- Providing reliable information, emotional support, and ongoing guidance to individuals and families navigating complex community and healthcare resources.
- Mailing patient correspondence, including monthly treat-and-release letters, and maintaining timely and accurate program documentation.
- Accompanying clients to appointments and community resources, as needed, using reliable personal transportation.

To Be Considered:
- Education: High school diploma required. Bachelor's degree preferred.
- Experience: Three (3) years of recent experience, including any combination of student, volunteer, community, or professional experience involving the Jersey City area and its community resources.
- Community Experience: Demonstrated interest in serving the Jersey City community required. Participation in community organizations and firsthand understanding of issues related to interpersonal violence preferred.
- Training: Successful completion of hospital-wide orientation, trauma-informed care training, and shadowing experiences with community outreach organizations required.
- Technical Skills: Basic computer and software application skills and the ability to perform basic internet searches required.
- Transportation: Must have reliable personal transportation to accompany clients to and from appointments and connect clients with community resources, as needed.
- Collaboration: Demonstrated ability to work independently, seek guidance when necessary, and function effectively as part of a multidisciplinary team.
- Communication: Strong listening, interpersonal, and communication skills required.
- Additional Experience: Previous mentoring experience desired.
- Environment: Sensitivity to working in a substance use environment preferred.
- Program Responsibilities: Supports the development and implementation of a community violence prevention program focused on underserved and low-income populations in the Greenville section of Jersey City, with the goal of reducing violence and improving community outcomes.
