RN Case Manager - Discharge Planning (3-Month Contract)

Pride Health is seeking an RN Case Manager for a client in Waukesha, WI on a 3-month contract. The role focuses on continuum of care planning, discharge assessment, patient education, and timely transfer coordination with the care team to ensure optimal outcomes.
