Clinical Coordinator II - RN

Position Summary The Department of Dermatology is seeking a Clinical Coordinator II / RN to join our team in a full-time position to work within the Division of Pediatric and Adolescent Dermatology. Columbia University Irving Medical Center’s Department of Dermatology’s Division of Pediatric and Adolescent Dermatology is a world leader in providing care for children and adolescents. Our mission areas include Healthcare, Education and Research. Our patients have acute care needs and utilize many services across our ambulatory and inpatient sites. This position will support the provision of high-quality, patient centered care for the Department of Dermatology by providing clinical support and care coordination to our Division of Pediatric and Adolescent Dermatology. Responsibilities Coordinate care for complex and high risk pediatric and adolescent patients with genetic disorders associated with skin disease (ichthyosis, epidermolysis bullosa tuberous sclerosis, neurofibromatosis), inflammatory and autoimmune disorders (atopic dermatitis, psoriasis, rheumatologic disorders), cancer related skin disease and vascular anomalies (hemangiomas, vascular malformation syndromes). Work with the pediatric dermatology physicians to develop knowledge of disease pathogenesis, prognosis, and treatment plans for various pediatric dermatologic disorders. Assess learning needs of the family and work with pediatric dermatology physicians to develop and implement education plan. Triage messages from patients/families in conjunction with pediatric dermatology team (Dermatology attending, nurse practitioner, medical assistant and fellow.) Coordinate outpatient pediatric dermatology appointments in collaboration with inpatient team Meet with inpatient care team and/or families when needed to develop follow-up plan of care, provide in-service and/or consultation to provider groups. Communicate with patients via telephone and secure messaging through medical record/patient portal to assist with care coordination. Coordinate with home care agencies, school nurses and primary care providers. Patient and family education and instruction regarding plan of care and at-home self-care. Coordination of care across multiple specialties across Columbia University Irving Medical Center (CUIMC) and New York Presbyterian- Morgan Stanley Children’s Hospital (NYP-MSCH). Organize/facilitate twice monthly Multidisciplinary Vascular Anomalies Clinic (VAC) sessions working alongside an interdisciplinary team that includes pediatric specialists from multiple disciplines including Dermatology, Hematology, Genetics, lnterventional Radiology, Otolaryngology, Plastic Surgery, General Surgery, and Radiology. Review the charts of these complex patients prior to their VAC appointments, aggregate their data, ensure they have gone for the appropriate recommended screening studies in advance and after appointments. Partner with pediatric dermatology leadership to support other pediatric interdisciplinary initiatives including (Epidermolysis Bullosa Interdisciplinary sessions) Provide wound care education, assist with pediatric dermatology procedures (e.g., laser treatments and first assist with excisions). Provide assistance with procedures requiring medication applications, teaching and monitoring. Lead and participate in quality improvement and patient experience initiatives improvements for Dermatology. Perform other related duties and responsibilities as assigned/requested. Minimum Qualifications Bachelor’s degree in Nursing or equivalent in education and experience, plus 4 years of related experience. BLS certified. Current New York State RN license. Preferred Qualifications Experience with Pediatric and/or Dermatology patients. Excellent communication skills. Bilingual (Spanish and English). Collaborate with physicians in the assessment, evaluation, treatment, and ongoing management of patients with dermatologic conditions. Perform surgical procedures including local anesthesia, punch and shave biopsies, suture removal, and wound care. Prepare, dilute, and draw up medications as needed for use in the faculty practice and Resident Clinic. Assist with dermatology medical procedures including laser therapy, photodynamic therapy, and cosmetic procedures. Assess patients using all available physical and diagnostic data, incorporating medical history, and evaluate patient status and progress with the current plan of care. Provide coverage and assess patients during and after selected therapies; perform CPR. Attend staff meetings, patient support meetings, and continuing education activities pertaining to the specialty as appropriate. Participate in all quality improvement/process improvement initiatives. Conduct comprehensive patient assessments, obtain accurate medical histories through effective interviewing techniques, ensure thorough and timely documentation in the medical record, and provide patient and family education regarding disease state, treatment plans, medications, side effects, and follow-up care. Perform phone triage as appropriate and provide follow-up guidance and direction to patients based on the situation. Participate in sign-out activities and patient follow-up. Ensure compliance with applicable regulatory agency standards (e.g., OSHA, JCAHO, DOH, HIPAA, billing compliance, etc.). Perform diagnostic testing and analysis of results. Order appropriate laboratory supplies, follow up on results, answer pre- and post-operative questions, gather data, and act as a liaison.
