Registered Nurse – Case Management

Schedule: Mon through Fri 8-5

Assignment Details:

• Contract Length: 13 weeks
• Guaranteed Hours: 40

Requirements:

• Active Registered Nurse license in the State
• Case Management experience within the last 2 years
• Proof of Flu Vaccine required

Roles & Responsibilities:

• Provide professional nursing care across the lifespan in accordance with organizational standards and evidence-based practice.
• Assess, plan, implement, and evaluate patient care, delivering high-quality, culturally sensitive, patient-centered services.
• Collaborate with providers and interdisciplinary teams to promote optimal patient outcomes and safety.
• Conduct nursing evaluations and coordinate care plans with primary and specialty providers.
• Facilitate medical care and medication refills for Behavioral Health/Psychiatry patients, collaborating with physicians and pharmacies.
• Provide education and support to patients related to care and discharge needs; advocate for patient care within the healthcare system.
