This role oversees referrals, intakes, and discharges, and manages the complex case team for admissions, referrals, and outreach purposes. A key responsibility is tracking referrals from the moment they are received until the team makes first contact, ensuring no client slips through the cracks. Within their assigned region, this manager also provides direct oversight to local ACT teams on access and transition fidelity. This position supports ACT teams in locations from Raleigh & Durham.Oversees the client journey into and out of ACT services, ensuring smooth coordination of referrals, intakes, admissions, transitions, and dischargesManages and monitors the ACT referral pipeline to ensure timely engagement, outreach, and follow-up from initial referral through first client contactSupervises and coordinates complex case team functions related to admissions, referrals, transitions, and outreach effortsEnsures no referrals or potential clients are lost in process gaps by maintaining oversight of referral tracking and engagement workflowsProvides direct operational oversight and support to assigned regional ACT teams related to access, intake processes, and transition fidelityMonitors and supports team productivity, staffing alignment, scheduling, and overall operational efficiency within assigned regionsOversees foundational operational structures to ensure compliance with administrative standards, staffing requirements, and operational expectationsThis description outlines the general scope and level of work for this role. It is not intended to include every duty or responsibility that may be required of employees in this position.CompensationThis role is compensated on a salaried basis, providing consistent earnings aligned with the responsibilities and expectations of the position.Placement within the range is based on licensure level, education, and experience.What You Can expectLead and support the referral, intake, admission, and discharge process for ACT services.Ensure clients are connected to services quickly by managing referrals and outreach efforts.Work closely with ACT teams across Gastonia and Charlotte to improve access and transitions of care.Help drive operational success by monitoring team workflows, productivity, and service coordination.ResponsibilitiesOversee referrals, intakes, admissions, transitions, and discharges to ensure a smooth client experience.Manage the referral pipeline and track client engagement from referral through first contact.Supervise and support teams responsible for admissions, outreach, and complex case coordination.Monitor operations, staffing, and workflows to ensure efficient services and compliance with program standards.QualificationsMaster’s degree in Human Services, Social Work, Psychology, Counseling, Nursing, Healthcare Administration, or a related field requiredMental health License in North Carolina (LCSW/A, LCMHC/A )Minimum of 1–2 years of leadership, supervisory, or program coordination experience preferredKnowledge of North Carolina behavioral health service definitions, Medicaid requirements, and community mental health systems preferredWorking knowledge of ACTT model expectations and familiarity with TMACT standards preferredAbility to oversee operational workflows, referral management, staffing coordination, and productivity expectationsStrong organizational, communication, and problem-solving skills with ability to manage multiple priorities effectivelyExperience working with interdisciplinary teams and coordinating care across providers and community stakeholdersAbility to review operational data, monitor compliance expectations, and support process improvement initiativesProficient in electronic health records (EHR), Microsoft Office, and operational reporting systemsValid North Carolina driver’s license with acceptable driving record requiredWhy Join UsWe are focused on creating a model where clinicians are supported, not overloaded.
