LMSW Case Manager

Kaiser Permanente seeks an LMSW Case Manager to deliver compassionate, patient-centered care within our Healthcare & Medical Services department. In this role, you will assess psychosocial needs, develop person-centered care plans, coordinate services, and advocate for patients across the continuum of care. You’ll collaborate closely with physicians, nurses, and community partners to support safe discharges, reduce readmissions, and optimize health outcomes. Drawing on a culture of innovation and collaboration, you’ll use data, EMR tools, and evidence-based practices to improve workflows, support quality metrics, and continually advance patient experience and access to care.
