POSITION: Home Health RN

LOCALS UNDER 100 MILES ACCEPTED. Anyone who lives less than 100 miles from the facility is limited to one assignment and no extension eligibility. Travelers who live less than 50 miles away are subject to the same rule, and a BR of *** less than posted

SCHEDULING DETAILS AND CHARTING
==============================
• Shift hours/Contract Length: M-F 8a-5p

• Charting: Epic experience required and OASIS assessment experience required.

• Required: Proficient in nursing assessment and OASIS data collection

• Weekend Requirement: No

• RTO: ALL requests must be included with the submission. Requests will only be accommodated if approved by the facility at the time of booking.

• Call off Policy: Facility is allowed to call off 1 shift per 2-weeks.

• Accept First-Time Travelers: Yes. Travel experience preferred.

• Locals Accepted? es. 0-49.9 miles--Bill rate would be *** less than posted BR. 50-99.9--same rate*** Approved for one 13 week contract.

REQUIRED LICENSE AND CERTIFICATIONS

• BLS

• Pending License Accepted: Yes, only if the candidate holds a single-state license from a non-compact state. Otherwise, submission must include an active Virginia State or Compact RN/LPN license.

REQUIRED CLINICAL SKILLS
========================
• 2+ years of Home Health experience required.

• Documentation : Admission assessment, Case Management, Discharge, Recertification, Transfer, Standard Visit

• Settings: Adult Home Health and Home Infusion

• IV : Care of peripheral IVs/Central Lines/Ports, phlebotomy, starting IVs

• Respiratory: Chest Tubes, Nebulizers, oxygen administration, trach care

• GI: Colostomy/Ileostomy care, feeding tubes, G-site care, NG tube care

• GU: Post AV fistula/shunt, suprapubic catheter insertion and care, urinary catheter insertion and care

• Medications: Intradermal, IM, IV infusions, PCA pump, Subcutaneous, pain scales

UNIT DETAILS AND FLOATING

• Daily patient volume-average visits per day-6

• Visits would include some rural areas--25 mile radius.

•Areas covered: Fincastle , New Castle, Daleville, Troutville, Roanoke City and Roanoke County, Salem, Catawba, Bent Mtn, Boones Mill ( Roanoke County side)

• Floating: Office is based out of Roanoke but may float to other offices as needed.
