Key Responsibilities\n

• Develop and implement individualized treatment plans based on comprehensive assessments of clients’ needs, strengths, and challenges.

• Conduct thorough interviews with clients to evaluate their current mental health status, emotional well-being, and potential risks.

• Coordinate and manage tailored support packages that facilitate client recovery and help them overcome dependencies.

• Monitor client progress continuously, adjusting treatment plans as necessary to ensure optimal outcomes.

• Provide crisis intervention services in acute situations, equipping clients with coping strategies and emotional support.
\n
• Maintain accurate and up-to-date documentation of client interactions, progress notes, and treatment recommendations in compliance with organizational standards.

• Engage in case conferences with multidisciplinary teams to collaboratively develop comprehensive care strategies.

• Act as a liaison by referring clients to appropriate community resources and service agencies, enhancing their access to necessary support systems.

Qualifications\n
