Key Responsibilities\n

• Conduct thorough psychosocial assessments through patient interviews, direct observations, and a comprehensive review of medical records, evaluating emotional and environmental factors affecting recovery.

• Offer emotional and practical counseling support to patients and their families, addressing adjustment issues and promoting psychological health throughout the rehabilitation process.

• Develop and implement individualized treatment plans, in collaboration with our interdisciplinary rehabilitation team, that address the psychosocial needs of each patient based on their assessments.

• Assist clients in navigating community resources, including support groups, vocational rehabilitation services, and housing assistance, ensuring a holistic approach to recovery.

• Provide education to patients and families about available community resources and transitional services following rehabilitation.

• Engage in discharge planning to facilitate smooth transitions for patients returning home or progressing to other care settings, ensuring continuity of support.
