Child and Family Focus, Inc. is seeking a Blended Case Manager in Pennsylvania to develop, coordinate, and implement services per the client’s service plan. You will provide case management, service planning, and coordination with other agencies, while offering advocacy to help clients reach recovery goals and increase self-advocacy.
