Work Environment

This position is designed for onsite work, specifically within patients' homes, allowing for a personalized approach in familiar surroundings. Our Home Care setting is structured to enable therapists to develop intimate, trusting relationships with clients, making it possible to tailor interventions that directly address their unique needs and goals. We provide our therapists with access to necessary equipment and resources to ensure effective treatment in the home environment.

Key Responsibilities

• Conduct comprehensive evaluations of patients in their home environment to assess mobility, strength, and functional abilities.
• Develop and implement individualized treatment plans that utilize evidence-based practices including exercise regimens, therapeutic modalities, and mobility enhancement techniques.
• Monitor and document patient progress diligently, adjusting treatment plans as warranted by clinical evaluations and patient feedback.
• Educate patients and their caregivers on rehabilitation strategies, including the use of assistive devices and self-care techniques to promote independence.
• Collaborate with multidisciplinary team members, maintaining clear communication about patient progress and care strategies.
• Stay current with developments in the field by participating in professional workshops, engaging with literature, and being active in professional organizations.

Patient Population / Client Focus

The Physical Therapist will work primarily with patients recovering from various injuries, surgeries, and chronic conditions. This diverse caseload may include pediatrics, geriatrics, and adult populations with orthopedic, neurological, or cardiopulmonary issues. The focus will be on enhancing functionality, providing pain relief, and implementing methods that allow each patient to achieve their maximum rehabilitation potential.

Qualifications

• Valid Doctorate in Physical Therapy (DPT) and an active state license in Physical Therapy.
• A minimum of two years of relevant clinical experience, with a strong understanding of home health methodologies.
• Excellent communication and interpersonal skills are essential for successful patient interactions and team collaborations.
• Proficiency in computer systems and electronic health records for tracking patient information.
• Demonstrated flexibility and adaptability in dynamic healthcare environments.

Preferred Experience

• Experience in home care or community health settings is preferred, but not required.

Compensation and Benefits

Carilion offers a competitive salary range of USD 90,000 - 110,000 annually, commensurate with experience. Our benefits package includes a sign-on bonus, flexible work schedules, and weekly pay via direct deposit.
