This position offers a fully remote work setting, allowing you to provide critical support without the need for onsite presence. You'll collaborate with a variety of healthcare professionals including behavioral health specialists and primary care doctors. Key Responsibilities\n

• Engage with patients, their families, and care teams effectively, employing active listening and clear communication tailored to their needs.

• Conduct brief, evidence-based screenings to identify behavioral health concerns, such as anxiety, depression, and substance abuse, while arranging comprehensive assessments as necessary.

• Create and manage a caseload of patients, providing timely follow-up interventions and linkage to additional treatment resources.
\n
• Utilize the electronic medical record (EMR) system diligently to facilitate integrated healthcare services, ensuring documentation is accurately completed within 48 hours of patient encounters.
\n
• Develop and implement integrated care plans in collaboration with other care team members, aligning services with the unique needs of each patient.
\n
• Uphold the highest standards of patient confidentiality and privacy in all communications, documentation, and data handling.
\n
• Participate actively in team meetings, supervision sessions, and professional development activities to foster personal and organizational growth.
\n
• Provide referrals to consulting psychiatrists when patients require psychiatric medication management, ensuring comprehensive care.
\n
• Return calls promptly to patients and other stakeholders, addressing concerns or appointment requests efficiently.
\n
• Proactively connect patients with community resources that align with their recovery goals, such as educational and vocational services.
\n
• Undertake other relevant responsibilities as needed to support organizational objectives.

Patient Population / Client Focus\n

Your primary focus will be on patients enrolled in our detox program, often from underserved communities with complex needs. You'll work closely with individuals facing addiction, mental health challenges, and social determinants of health, striving to equip them with the support and resources necessary for achieving recovery and improving their overall well-being.

Qualifications\n

• Master’s degree in Social Work or a closely related behavioral health field.

• Active LCSW licensure is required to practice.
\n
• At least three (3) years of relevant experience in the behavioral health field.

• Current CPR or BLS certification is essential.

• Demonstrated cultural competence, particularly with underserved populations and individuals experiencing homelessness.

• Experience in utilizing trauma-informed care and social justice frameworks in practice is preferred.

• Familiarity with the use of electronic medical records for documentation and information retrieval.

• Proficiency in Spanish is highly desirable to enhance communication with our diverse patient population.
