Travel to various CHDC locations may be required for meetings, professional development sessions, and outreach activities.

Key Responsibilities\n

As a Licensed Clinical Social Worker at CHDC, your role entails critical responsibilities that directly impact patient outcomes. Key duties include:

• Delivering quality clinical behavioral health services that align with CHDC's mission, enhancing both patient satisfaction and health outcomes.

• Assisting clients in navigating both physical and behavioral healthcare systems to ensure integrated and comprehensive care.

• Educating patients about available community resources to foster informed participation in their health journey, promoting disease prevention and wellness practices.
\n
• Engaging in and contributing to multi-disciplinary treatment team meetings, sharing insights and strategies to optimize patient care.
\n
• Promptly reporting any crisis situations and unusual incidents to the Medical Director, with immediate notification of psychiatric or medical crises to the Chief Medical Officer.
\n
• Maintaining precise documentation in clinical records, ensuring compliance with legal and organizational standards.
\n
• Actively participating in the Quality Improvement program through ongoing evaluation of care practices and attending assigned training and meetings.
\n
• Utilizing evidence-based screening tools to evaluate healthcare needs, interpreting results to inform treatment plans.
\n
• Working closely with our Center of Excellence Team to support patients with Opioid Use Disorder, facilitating their path to recovery.

Patient Population / Client Focus\n

The primary focus of this role will be on individuals within the detox environment, many of whom may be experiencing complex behavioral health challenges, including substance use disorders. Our goal is to provide empathetic, evidence-based support that empowers patients to achieve healthier lifestyles while addressing their mental health needs comprehensively.

Qualifications\n

To be considered for this role, you must possess the following qualifications:

• A Master’s degree in Social Work, with a current, valid Licensed Clinical Social Worker (LCSW) certification.

• At least 2-3 years of relevant experience in clinical social work, particularly in a healthcare setting.

• A demonstrated ability to collaborate effectively with a multidisciplinary team, including healthcare professionals and client advocates.

• Excellent communication skills, with a proven capacity to educate and empower clients.

• Willingness to travel as needed to various CHDC sites for meetings, workshops, and outreach initiatives.
