Key Responsibilities\n

• Conduct comprehensive evaluations and assessments to understand the psychosocial needs of patients and their families.

• Participate actively in the development and implementation of Individualized Family Service Plans (IFSPs) aimed at enhancing the quality of life for patients.

• Monitor and evaluate treatment outcomes, coordinating with interdisciplinary teams to ensure aligned care goals.

• Provide family support services by offering practical resources and emotional guidance tailored to specific family dynamics and needs.

• Adhere to all applicable Federal and State regulations relevant to social work practice.

• Build trusting relationships with families and patients, fostering a therapeutic alliance that encourages open communication and collaboration.

• Facilitate access to community resources, completing referrals as necessary to support both immediate and long-term patient needs.

• Utilize coaching techniques and evidence-based practices to empower families in enhancing their child’s developmental skills.

• Remain compliant with health and safety standards to ensure the welfare of all individuals receiving services.

Patient Population / Client Focus\n

The primary focus of this position will be on patients receiving hospice care, including children with developmental delays or disabilities. Our LCSWs work to provide both emotional and practical support to these families, ensuring they have the necessary guidance throughout the care process. This role is instrumental in addressing the unique challenges faced by families during end-of-life scenarios, requiring sensitivity and professionalism to navigate complex emotional landscapes.

Qualifications\n

• A Master’s degree or Doctorate in Social Work from an accredited institution.
