Grow your career while you help make a better future for our clients and families.As a SEA employee, you’ll enjoy a competitive benefits package and programs including, but not limited to:Comprehensive health, dental and vision insurance optionsFlexible Spending Account and Commuter Spending AccountEmployee Assistance Plan for work life balanceRetirement Savings Plan (403b) and matchingSupplemental Insurance OptionsLife InsurancePaid Time OffPaid HolidaysCorporate DiscountsPOSITION SUMMARY:The Case Manager is responsible for providing trauma-informed, culturally responsive support, crisis intervention, and resource navigation for individuals and families impacted by community violence.DUTIES AND RESPONSIBILITIES:Trauma-Informed Assessment by conducting strength-based assessments addressing histories of community trauma, violence exposure, and co-occurring challenges.Trauma-informed care by building trusting relationships honoring cultural humility and psychological safety.Care Coordination by linking clients to wrap-around resources, including mental health counseling, substance use support, legal assistance, housing aid, tattoo removal, employment readiness, etc.Service Planning by developing individualized service plans focused on safety, behavioral health, stable housing, and educational and employment goals.Community Outreach by collaborating with multidisciplinary partners including trauma centers, law enforcement liaisons, local intervention workers, and community organizations.Crisis Intervention by proving immediate de-escalation, risk assessment and safety planning during critical incidents.Documentation and Compliance support by maintaining accurate, timely case notes and tracking client activities and outcomes, community engagement and other data collection required by the contract for service delivery.Prepare monthly case-related reports including outcomes, successes and challengesAdvocate for clients with agencies and service providers to minimize barriers to clients receiving needed servicesEngage and collaborate with parents, guardians, siblings, and other family members to assess needs, provide supportive services, and strengthen family stability and client outcomes.Actively participate in staff meetings, trainings, client reviews, internally and with partner agenciesMaintain confidentiality and comprehensive confidential case filesEstablish and maintain collaborative working relationships with community resourcesPerforms related duties as required. **Related duties as required are duties which may not be specifically listed in the class specification or position description, but which are within the general occupational series and responsibility level typically associated with the employee’s class of work.Any combination of experience and education likely to provide the required knowledge, skills and abilities, typically:Bachelor’s degree in a related field1 to 3 years of experience in related field2 years of experience working with high risk or probation youthCommitment to the mission and values of the non-profit sector, with a passion for social impact and community serviceMust have reliable transportation, a valid CA Driver’s License, and proof of auto insuranceMust be able to successfully pass Live Scan (background check)Required TB Test clearanceKNOWLEDGE, SKILLS, AND ABILITIES:Knowledge of:Outreach and engagement strategiesCommunity resourcesHuman behavior and social scienceSkills and Abilities:Ability to work in diverse settings and provide culturally competent servicesAbility to successfully balance time in the community with clients with the documentation requirements of the programMust have patience, listening skills, and professionalismDisplay sensitivity, tact, and responsiveness in various adverse situations and needsCapacity to work collaboratively as a team memberGood written and verbal communication skillsMaintain high level of confidentialityHandle public contact and relations effectivelyDevelop and maintain effective working relationships*Soledad Enrichment Action strongly values diversity, equity, and inclusion and is committed to equal opportunity in employment.
