Remote Care Manager for Complex Health & Social Needs

Registered Nurse

UnitedHealthcare is seeking a Care Coordinator to manage a panel of dual eligible members with chronic and complex health needs. The role focuses on person-centered care planning and coordinating medical, behavioral, and social needs to improve outcomes.

This fast-paced position requires multitasking, attention to detail, and strong organizational skills. Remote work is available within specified Idaho counties with flexible scheduling.
