Nurse Practitioner or Physician Assistant, Northern Syracuse -Care at Home

$40,000 Student Loan Repayment OR $30,000 Sign‑On Bonus In Home Care, No Nights, Weekends or Holidays This position requires daily travel to patient homes within the assigned area. Primary Responsibilities Conduct comprehensive in‑home assessments, including past medical history review, medication reconciliation, vital signs, and physical exam. Perform evidence‑based screenings and point‑of‑care testing as appropriate. Manage members through acute or exacerbation of chronic conditions, including testing, diagnostics, and interventions. Manage members during short‑term sub‑acute rehabilitation episodes. Ensure accurate and complete documentation, including ICD‑10 coding. Communicate and collaborate with the interdisciplinary care team. Conduct advanced illness and advanced care planning conversations. Provide patients and caregivers with counseling and education. Required Qualifications NP: Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult‑Gerontology Primary Care. PA: Current NCCPA certification and state licensure. Active, unrestricted licensure in the state of practice (or ability to obtain by start date) without disciplinary actions. Active and unrestricted driver’s license and reliable transportation for travel. Current active DEA licensure/prescriptive authority or ability to obtain post‑hire. Ability to gain a collaborative practice agreement, if applicable in your state. Ability to navigate varied environments and ability to sit, stand, kneel as needed to perform duties. Preferred Qualifications 1+ year clinical experience as an APC (family, geriatric, or home health preferred). Experience in geriatric medicine, long‑term care, senior living, or home care settings. Experience meeting the medical needs of patients with complex behavioral, social, and/or functional needs. Understanding of geriatrics and chronic illness. Understanding of advanced illness and end‑of‑life discussions. Proficiency with electronic medical records and technology. Compensation & Benefits Compensation ranges from $125,000 to $175,000 total cash, including base pay and bonuses.
