Work Environment

This full-time position involves onsite work within an assisted living facility, characterized by a welcoming atmosphere dedicated to promoting the health and well-being of our residents. The Pain Management Center operates Monday through Friday from 8 AM to 4 PM, providing a structured schedule conducive to work-life balance. Key Responsibilities

• Conduct thorough patient assessments by obtaining detailed medical histories and performing physical examinations on patients assigned by the supervising physician.
• Independently manage follow-up visits, ensuring adherence to the organization’s guidelines and protocols.
• Maintain accurate patient records, meticulously verifying information at each appointment to support patient care coherence.
• Analyze diagnostic test results, promptly determining the necessity for follow-up appointments or additional treatment interventions.
• Collaboratively develop personalized medical care plans under the guidance of a physician, recommending treatment options and consistently evaluating patient progress and responses to therapies.
• Deliver quality care while achieving established company productivity metrics and maintaining high standards of practice.
• Act as an educational resource for patients, providing ongoing guidance and support related to their treatment regimens and health management strategies.

Patient Population / Client Focus

In this role, you will primarily serve an older adult population residing in the assisted living facility. Many residents may suffer from chronic pain conditions, necessitating specialized knowledge in pain management. Your work will directly impact their quality of life by ensuring effective pain control and support for their overall health needs in a compassionate and empathetic manner.

Qualifications

• A Master's Degree in Nursing, specifically as a Nurse Practitioner, obtained from an accredited program.
• A valid Nurse Practitioner license to practice in New York State.
• A minimum of 2 years of clinical experience in pain management or conducting physical examinations.
• Excellent clinical skills coupled with strong interpersonal and organizational abilities, demonstrating a commitment to teamwork.
• Competence in using Electronic Medical Records (EMR) and/or Electronic Practice Management systems.
• Proficient in Microsoft Office applications, particularly Word and Excel.
• An ability to thrive in a fast-paced environment, demonstrating resourcefulness, adaptability, and proactivity in patient care.

Preferred Experience

While not mandatory, experience with older adult populations, particularly in assisted living or rehabilitation settings, will be highly advantageous. Familiarity with interdisciplinary care teams and integrated health approaches is also preferred.

Compensation and Benefits
