Work Environment

As a Nurse Practitioner in our assisted living facility, you will work closely with an experienced team of healthcare professionals, including those trained in physical medicine and rehabilitation, pain medicine, interventional pain management, and sports medicine. The setting is both supportive and structured, allowing you to focus on your clinical role while collaborating with counselors and diagnostic support staff to ensure that our clients receive well-rounded care.

Key Responsibilities

• Conduct comprehensive assessments of patients suffering from chronic pain to develop individualized treatment plans.
• Design and implement effective treatment modalities, incorporating medical interventions and rehabilitation strategies according to best practices.
• Manage and adjust medication regimens, employing careful monitoring to optimize efficacy while minimizing side effects.
• Educate patients and their families about pain management techniques and therapeutic exercises to facilitate active participation in their recovery.
• Regularly monitor patient progress through follow-up evaluations, adjusting care plans as needed based on feedback and therapeutic outcomes.

Patient Population / Client Focus

The primary focus of this role is to support elderly residents and individuals at our assisted living facility who are grappling with chronic pain conditions. Qualifications

• Current certification as a Nurse Practitioner is required.
• Active licensure to practice as a Nurse Practitioner in New York State.
