Work Environment

As a Nurse Practitioner in our home care program, you will engage with patients in their residences, allowing for personalized consultations and care plans. Key Responsibilities

• Conduct thorough clinical evaluations of patients, assessing cardiac health and overall wellness.
• Develop, implement, and manage individualized treatment plans for patients with cardiovascular conditions.
• Perform initial physical examinations and patient observations to establish baselines and monitor progress.
• Operate, maintain, and monitor various medical equipment critical to patient care.
• Order and administer diagnostic tests, interpreting results to facilitate timely decision-making regarding treatment options.
• Communicate test results effectively to patients, educating them about their conditions and treatment plans.
• Document patient medical histories comprehensively to ensure continuity of care and adherence to best practices.
• Make appropriate referrals to specialists or additional healthcare providers when necessary, ensuring coordinated care.

Patient Population / Client Focus

The primary focus of this role will be on adult patients suffering from various cardiovascular diseases. You will work closely with individuals who may experience chronic heart conditions, hypertension, and related comorbidities. Our goal is to empower patients with knowledge about their health and collaborate with them on effective management strategies, enhancing their quality of life.

Qualifications

• A Master’s degree in Nursing from an accredited institution.
• Current certification as a Nurse Practitioner, with a specialization in Cardiology preferred.
• An active nursing license in the state of New York.
• Minimum of 2 years of clinical experience as a Nurse Practitioner, with a strong preference for experience in cardiology or related fields.

Preferred Experience

• Experience working within home care settings or community health
• Proficiency in the use of electronic medical record (EMR) systems to document and track patient care.
• Familiarity with guidelines and best practices in cardiology.
