Work Environment

This full-time position is based onsite in a home care environment, where you will visit patients in their homes, assessing their individual needs and comfort. Working closely with a multidisciplinary team, including physicians, nurses, and social workers, you will help design and implement comprehensive care plans. Key Responsibilities

• Conduct thorough health assessments and physical examinations for patients of all ages in the comfort of their homes, enabling tailored diagnosis and treatment plans.
• Order, interpret, and follow-up on diagnostic tests such as bloodwork and imaging studies, ensuring timely interventions and adjustments to care.
• Prescribe necessary medications and perform therapeutic procedures, adjusting prescriptions based on patient responses and evolving health conditions.
• Deliver impactful patient education sessions, focusing on health maintenance, disease prevention, and management plans for chronic conditions such as diabetes, hypertension, and more.
• Collaborate dynamically with other healthcare team members to coordinate optimal care pathways, facilitating effective referrals, and ensuring continuity of care across various settings.
• Document patient interactions meticulously, maintaining accurate medical records aligned with regulatory and organizational standards to ensure compliance and quality care delivery.

Patient Population / Client Focus

You will be working primarily with adult and geriatric patients who may have chronic health issues, mobility challenges, or require supportive care in their homes. Your understanding of the complexities of home care will be vital, as will your ability to foster trusting relationships that encourage compliance and engagement in their health journeys.

Qualifications

• An active and current Nurse Practitioner license in New York State.
• A Master's or Doctoral degree in Nursing from an accredited program.

Compensation and Benefits

We offer a competitive salary range of USD 150,000 to 160,000 annually, in addition to a comprehensive benefits package that includes health insurance, retirement plans, paid time off, and continuous education opportunities.
