Jackson Medical Center, located in the heart of Rome, New York, is seeking a dedicated and passionate Family Nurse Practitioner to join our team in a full-time capacity. Work Environment

The work setting for this position is onsite and primarily involves home care visits within the community. Operating within a full-time framework, you will collaborate with a multidisciplinary team that includes physicians, social workers, and health educators, all committed to enhancing health and well-being in Rome and its surrounding areas.

Your standard working hours will be from 9:00 a.m. to 6:00 p.m., with potential evening clinics extending until 8:00 p.m. Participation in community outreach events and health fairs, which may occasionally occur over weekends, is expected. This role offers an average workweek of approximately 40 hours, promoting a balance between professional commitment and personal time.

Key Responsibilities

• Conduct detailed health assessments and screenings for patients within the community, collecting necessary information to inform diagnoses and treatment plans.
• Effectively diagnose, treat, and manage both acute and chronic health conditions, while also providing preventive care and health promotion strategies.
• Collaborate with local community organizations, public health officials, and stakeholders to design and implement health education initiatives aimed at raising awareness around prevalent health issues.
• Utilize cultural competence to deliver personalized care, advocating for patients’ rights and ensuring equitable access to healthcare services.
• Coordinate patient referrals and follow-up care, ensuring that patients receive the necessary support along their healthcare journey.
• Maintain accurate, thorough medical records, ensuring compliance with all relevant regulatory standards and best practices.
• Participate in community outreach activities, such as health fairs, to engage with the public, provide screenings, and promote health literacy.
• Keep abreast of current community health trends, evidence-based practices, and policy changes that may impact service delivery.

Qualifications

• Possession of a current Nurse Practitioner certification and a valid state nursing license.
• A Master’s degree in nursing from an accredited institution is required.

Compensation and Benefits
