Senior Clinical Pharmacist - Anticoagulation

The Senior Clinical Pharmacist will collaborate with physicians and staff to provide coordinated, comprehensive, high-quality care to patients. The responsibilities of the clinical pharmacist will predominantly fall into the following areas:

• 1) Patient Care - The Senior Clinical Pharmacist is responsible for providing collaborative practice services in collaboration with the healthcare team.
• 2) Population Management - The Senior Clinical Pharmacist may utilize registries for chronic diseases and reports/tools to identify target populations/patients and enhance therapeutic outcomes through disease/medication education and drug therapy optimization.Providence caregivers are not simply valued – they’re invaluable. PharmD in PHARMACY from an accredited School of Pharmacy. Coursework/Training: Postgraduate Year One (PGY-1) Pharmacy Residency or equivalent education/experience.Upon hire: Washington Pharmacist License.Within 2 years of hire: Certification from Board of Pharmacy Specialties.Upon hire: Board of Pharmacy Preceptor License - when eligible and where applicable.3 years Pharmacy practice experience with at least 2 years of related experience in lieu of Postgraduate Year One (PGY-1) Pharmacy Residency.Why Join Providence?
