At Longleaf Hospice and Palliative Care, we integrate and address the diverse, seen and unseen needs of patients and their families.This position involves patient assessment, carrying out the established individualized plan of care and communication with physician to meet patient/caregiver needs: The PRN RN will:Use health assessment data to support the Plan of Care established by the RN Case Manager.; inform the patient and the family in the planning processMaintain current, accurate, detailed reports in accordance with policyRecord patients' medical information and vital signs. Regularly reevaluate patients' nursing needs when assigned to the patientMonitor, record and report symptoms and changes in patients' conditionsPrepare clinical notes and update the primary physician when necessary; obtain/receive physician orders as requiredCounsel the patient and family in meeting nursing and related needsProvide health care instructions to the patient as appropriate per assessment and planParticipate in on-call dutiesOther duties as assignedRequirements:Current license to practice as a registered professional nurse in GeorgiaCurrent and continued CPR certificationAt least one or two years of recent home care experience; hospice experience highly preferredGood verbal and written communication and problem-solving skillsResponsible valid driver's licenseReliable, insured, automobile for making home visits required
