Key Responsibilities\n

• Conduct thorough clinical assessments, gathering vital information to inform effective treatment plans tailored to each patient’s unique needs.

• Utilize evidence-based practices, including cognitive-behavioral therapy (CBT) and dialectical behavior therapy (DBT), to address a wide variety of mental health concerns.

• Maintain accurate and timely clinical documentation, ensuring compliance with HIPAA regulations and organizational standards.

• Collaborate with psychiatrists, psychologists, and a multidisciplinary healthcare team to provide comprehensive patient care.

• Participate in regular case consultations and peer review sessions to promote continuous improvement in the quality of mental health services.

• Educate patients and their families about mental health issues, treatment options, and available community resources to foster informed decision-making.

• Stay abreast of the latest research and advancements in mental health care to integrate best practices into your clinical approach.

Patient Population / Client Focus\n

In this role, you will primarily support individuals facing serious health challenges within a hospice setting, often dealing with grief, anxiety, depression, and existential concerns. Your expertise will help patients and families navigate the complex emotional landscape associated with end-of-life care, ensuring they receive compassionate support during one of the most challenging times of their lives.

Qualifications\n

• A Master's degree in Social Work from an accredited institution.

• Active and valid LCSW license in the state of California.

• Minimum of 2 years of clinical experience, ideally in a mental health or hospice environment.

Compensation and Benefits\n
