Intensive Care Coordinator

Job Overview Intensive Care Coordinator – Umatilla County Reports to Clinical Supervisor. Full‑time, FLSA exempt. Pay grade B8 ($24.95–$35.96 hourly depending on experience). Spanish bilingual preferred with 10% increase after language test. Relocation benefit for moves over 100 miles. 2‑year commitment with staged tax‑payout. Hiring bonus $5,500. The role involves coordinating mental health services and case management for children in need of intensive care coordination (ICC). The position works closely with community partners, facilitates referrals, manages ICC cases, and ensures compliance with Oregon Administrative Rules. Assists referrers with collection of information for referral packets and serving as point of contact. Facilitates communication among families, providers, natural supports, resource agencies. Provides orientation to ICC services. Organizes and facilitates Community Resource Team (CRT) meetings. Ensures continuity of care by creating linkages and managing transitions between levels of care. Assists with coordination and tracking of children in inpatient care. Coordinates reviews of all ICC cases every 30 days. Collects and submits data to GOBHI, REDCAP, and OHA. Promotes equality, diversity, and rights. Provides case‑management services to children, adults, and families. Acts as liaison and client advocate when appropriate. Participates actively in group and individual supervision sessions. Works with staff and community representatives to initiate and revise programs. Performs joint case planning with community agencies. Provides behavioral health and addiction education to groups. Handles civil commitment process as required. Completes paperwork professionally and timely. Attends trainings, meetings, conferences as directed. Assists other clinicians with group facilitation. Utilizes agency software to prepare client records and reports. Transports clients as required. Reports abuse and completes required paperwork and investigations. Other responsibilities as assigned. Qualifications Bachelor’s degree in social work, psychology, or related field (or equivalent combination of education and work experience). Knowledge of mental illness and addiction symptoms, and of relevant Oregon Administrative Rules. Excellent verbal and written communication skills. Ability to work independently and complete tasks within deadlines. Skilled at coordinating services across agencies and community groups. Ability to build respectful relationships with clients and families. Ability to represent client and agency interests favorably in community. Teamwork skills and strong interpersonal abilities. Computer literacy and proficiency with word processing and agency systems. Experience handling highly confidential information with adherence to confidentiality laws. Background Checks Must pass all criminal history checks as required by ORS 181.536‑181.537 and OAR 410‑007‑0200 through 410‑007‑0380. Ongoing monthly checks against federal and state exclusion lists. Exclusion or sanction leads to immediate termination. Pre‑hire Drug Screening Required for all applicants. Personal Auto Insurance Must hold a valid driver’s license and personal auto insurance for vehicles used for CCS business. Proof of minimum liability coverage of $300,000 bodily injury and $100,000 property damage is required. Physical Demands Essential duties include use of computer and phone systems, typing, reaching, and handling. Requires sitting, talking, hearing, occasional standing, walking, reaching, stooping, kneeling, and lifting up to 30 lbs. Vision required: close, peripheral, distance, and focus adjustment. Work Environment Work performed in offices and community settings. Moderate office noise, occasional loud noise, variable community environments, required travel, and overnight stays.
