Physical Therapist I - AVN, Per Diem, Sussex County Morristown, NJ, United States Job Description Atlantic Health is seeking an experienced Per Diem Physical Therapist for the Atlantic Visiting Nurse Department to work 8 am–4 pm in Sussex County. The role involves evaluating and treating patients in their homes, providing independence and autonomy in a therapeutic setting. Essential Functions/Responsibilities Evaluate and treat patients in their homes. Provide physical therapy interventions tailored to each patient’s needs. Collaborate with nursing and rehabilitation teams to develop care plans. Document progress and outcomes in the medical record. Adhere to safety and infection control standards. Qualifications High School Diploma or GED (required). Bachelor’s in Physical Therapy (required); Master’s or Doctorate preferred. At least 1 year of experience in home‑care settings (strongly preferred). Valid NJ License in Physical Therapy. Valid and unrestricted driver’s license with clean driving record (required). BLS certification (preferred). Location & Compensation Location: 465 South St, Morristown, NJ, 07960. Hourly rate: $41.79 – $73.56. Shift: Day, Mon‑Fri. Department: Home Care – Therapy.
