Physical Therapist I: Atlantic Health AVN, Full Time, Essex County

Atlantic Health is seeking a dynamic and motivated individual to join the energetic and innovative team of healthcare professionals in their Atlantic Visiting Nurse Department as a Full‑Time Physical Therapist to work 8AM-4PM including one weekend every four to six weeks in the Essex County area. The ideal candidate will evaluate and treat patients in the patient’s home and be a part of a collaborative team of health providers. Atlantic Visiting Nurse provides personalized care for you or a loved one to regain health, stay out of the hospital, and remain living independently at home. Whether or not it is managing life with a chronic illness, living with a disability, returning home from the hospital or rehabilitation facility, or needing assistance with end‑of‑life care, we are available to you in six counties throughout New Jersey: Morris, Somerset, Essex, Sussex, Union, and Warren. Responsible for providing a broad range of physical therapy services in an interdisciplinary setting, including assessment, treatment planning, and therapeutic interventions in accordance with the position’s qualifications, professional practices, and ethical standards. Ensures and provides development, quality improvement, problem‑solving, and productivity enhancement in a flexible interdisciplinary manner. Principal Accountabilities Perform initial examinations and differential diagnosis. Provide and prescribe plans of care consisting of corrective therapeutic exercise. Perform manual treatments consisting of soft tissue techniques, joint mobilizations, and many more. Supervise and interact with support staff and maintain proper documentation. Evaluate and establish a plan of care and treat patients accordingly. Document patient care and charges appropriately, and document the course of patient care, including progress made and the ongoing need for treatment. Work with other professionals in an effective manner. Provide clinically appropriate therapeutic interventions. Evaluate patient response to treatment and provide feedback to the interdisciplinary team. Perform other related duties as assigned. #J-18808-Ljbffr Atlantic Health
