The Department of Developmental Services is seeking an enthusiastic, compassionate and dedicated Registered Nurse to fill the Registered Nurse IV (RN IV) position in the Springfield/Westfield Area Office. The RN IV functions as a Health Care Coordinator for the Area Office and promotes health and safety by providing professional nursing consultation, education, training, resource identification and advocacy to individuals, families, vendors and DDS provider staff in the sixteen (16) cities and towns served in the Springfield/Westfield area. Duties and Responsibilities (these duties are a general summary and not all inclusive): The Registered Nurse IV participates in clinical assessments; risk reviews and tracking; nursing home evaluations and diversion planning; death reports and mortality reviews; medication reviews in accordance with Medication Administration Program (MAP); consultation on aging, Alzheimer's and end of life issues; and other reviews and activities for individuals with high medical needs. RN IV works closely with the Area Office staff to plan and resolve issues involving individuals at high-risk due to complex medical needs. The RN IV acts as a liaison to other medical and clinical professionals and develops relationships with area hospitals and medical providers. The RN IV provides consultation and assistance to provider based nursing staff and facilitates communication and coordination with medical and psychiatric providers who may accompany individuals on visits to these professionals. The RN IV acts as a liaison during acute hospitalizations gathering and providing information through hospital visits, staff discussion and discharge planning meetings and home visits, particularly with the legal department and management team on end of life. The Registered Nurse IV will function as a member of the Area Risk Management Team; assess-medical, nursing, psychiatric issues and work with the Area Office Team to coordinate clinical services for individuals, write reports, and recommendations as needed; as well as perform other related duties as requested and assigned by the Area Director or designee. The RN IV will also provide individual-specific training and healthcare case review. Please Note: This position is located in the Springfield/Westfield Area Office, traveling to hospitals, nursing homes, group homes, etc., as the need arises across the region. Schedule: FT 37.5 hr position Days Off: Sunday/Saturday Flexibility required to meet the needs of the program/area office.

Qualifications: First consideration will be given to those applicants that apply within the first 14 days. MINIMUM ENTRANCE REQUIREMENTS: Applicants must have at least (A) two years of full-time, or equivalent part-time, experience as a registered nurse in a recognized hospital, clinic or medical facility and (B) of which at least one year must have been in a supervisory, administrative or managerial capacity or (C) any equivalent combination of the required experience and the substitutions below. Substitutions: I. A Graduate degree with a major in Nursing may be substituted for a maximum of one year of the required (A) experience.* No substitutions will be allowed for the required (B) experience. *Education toward such a degree will be prorated on the basis of the proportion of the requirements actually completed. SPECIAL REQUIREMENTS FOR ALL LEVELS IN SERIES: Current and valid registration as a professional nurse under the Massachusetts Board of Registration in Nursing.* *Recent graduates from an approved school of nursing must have applied for the first available registered nurse examination. Based on assignment, possession of a current and valid Massachusetts Motor Vehicle Operator's license may be required.

Salary placement is determined by a combination of factors, including the candidate's years of directly related experience and education, and alignment with our internal compensation structure as set forth by the Human Resources Division's Hiring Guidelines. For all bargaining unit positions (non-management), compensation is subject to the salary provisions outlined in the applicable collective bargaining agreement and will apply to placement within the appropriate salary range.
