This role is designed for a remote work setting, allowing for flexibility and adaptability in engaging with clients and community resources. While primarily home-based, our social worker will engage in regular virtual consultations and communication with clients, their families, and healthcare professionals. The Social Worker may also conduct in-person visits as necessary, emphasizing the importance of personalized care in our services. Regular team meetings and training sessions will be conducted virtually to maintain harmony and collaboration among our team members.

• Conduct comprehensive assessments of seniors’ psychosocial needs through interviews and standardized assessment tools, identifying emotional, social, and health-related factors impacting their daily lives.

• Develop and implement individualized care plans in collaboration with clients and their families, focusing on actionable goals that respond to their specific needs and aspirations.

• Provide counseling and emotional support aimed at facilitating healthy coping mechanisms during life transitions such as loss, illness, or changes in living conditions.

• Advocate for clients’ rights, ensuring they have access to essential community resources, services, and benefits tailored to enhance their quality of life.

• Work alongside interdisciplinary teams, including nurses, caregivers, and mental health professionals, to coordinate care and address complex client situations, ensuring a holistic approach to their well-being.

• Regularly visit clients to monitor their health and safety, assess evolving needs, and promptly address any concerns through direct communication.

• Educate clients and families about local resources, organized support groups, and available programs designed to foster community engagement and social connectedness.

• Document client assessments, care plans, and progress notes with precision using electronic health record (EHR) systems, maintaining confidentiality and compliance with regulatory standards.

• Stay updated on best practices in social work, continuing education requirements, and local/state social work regulations, ensuring adherence to ethical guidelines.

The primary focus of this role is seniors, who may face various psychosocial challenges due to aging, health conditions, or life transitions. Our clients often require support related to mental health, emotional distress, family dynamics, and access to community services. By addressing these complex needs, our Social Worker contributes significantly to their overall well-being and empowers them to maintain independence and dignity.

• A Master's degree in Social Work (MSW) from an accredited institution is required.

• Must hold a current and valid state license as a Master Social Worker (LMSW).

• A minimum of two years of professional social work experience, preferably within a geriatric or community health setting.

• In-depth knowledge of aging-related issues, social work principles, community resources, and best practices is essential.

• Demonstrated proficiency in assessment, counseling, and case management skills, with the ability to connect with clients empathetically.
