Occupational Therapist

A permanent part-time home health occupational therapist role in the north of Stockton, California, offering flexible scheduling, competitive pay, and mileage reimbursement. The position focuses on meaningful one-on-one patient care and building lasting relationships in patients' homes.

Key Responsibilities:

• Evaluate patients' functional abilities and develop individualized treatment plans.
• Provide skilled occupational therapy services in patients' homes.
• Improve patients' independence with ADLs and IADLs.
• Assess home environments and recommend adaptive equipment or modifications.
• Educate patients, caregivers, and family on treatment plans and safety.
• Collaborate with physicians, nurses, PT, speech therapists, and other professionals.
• Maintain accurate, timely, and compliant clinical documentation.
• Monitor patient progress and adjust treatment plans as needed.
• Ensure compliance with all federal, state, and agency regulations.

Qualifications:

• Graduate of an accredited Occupational Therapy program.
• Current California Occupational Therapist license in good standing.
• Current CPR/BLS certification.
• Valid driver's license, reliable transportation, and current auto insurance.
• Minimum of one year of Occupational Therapy experience.
