Home Health Medical Social Worker (MSW) – Per Visit / PRNHope at Home Health Care is a Medicare-certified home health agency based in Southfield. We serve patients throughout Oakland, Macomb, Wayne and Livingston Counties. Our interdisciplinary team helps patients recover safely at home after a hospitalization, an illness or an injury.We’re looking for a Medical Social Worker to make home visits to our patients on a flexible, per-visit basis. You’ll work alongside our nurses and therapists to address the social, emotional and financial issues that stand in the way of a patient’s recovery and independence.Complete psychosocial assessments in patients’ homes and help develop the plan of careConnect patients and families with community resources: Medicaid, Medicare, MI Choice waiver, Area Agencies on Aging, Meals on Wheels, transportation, housing and caregiver supportProvide short-term counseling for adjustment to illness, caregiver stress, grief and end-of-life decisionsAssist with advance directives, long-term care planning and placementIdentify and report concerns about safety, abuse, neglect or self-neglectDocument visits accurately and on time in Kinnser/WellSkyCommunicate with the clinical manager and the interdisciplinary team, and take part in case conferencesMaster’s degree in Social Work (MSW) from a CSWE-accredited programActive Michigan LMSW licenseAt least 1 year of social work experience in a health care settingValid driver’s license, reliable transportation and auto insuranceCurrent CPR/BLS certificationExperience in home health or hospiceKnowledge of Medicare home health and community resources in Southeast MichiganExperience with an EMR (Kinnser/WellSky a plus)$70-$80 per visitFlexible PRN scheduling: you choose your availability and coverage area
