Acute Care Case Manager

Assignment Duration

This is a 13-week contract position.

Schedule

Day Shift: 5x8-Hour (08:00 - 16:30); Weekend Requirement: Every other weekend.

Job Description

We are seeking a skilled Acute Care Case Manager to join our team. The ideal candidate will have experience in case management and discharge planning within an acute care setting. The role involves coordinating patient care, facilitating discharge, and ensuring compliance with state and federal regulations.

Responsibilities

• Assess and manage a caseload of up to 25 patients, coordinating care plans and ensuring optimal patient outcomes.
• Participate in the development of treatment plans and family meetings.
• Conduct utilization reviews and ensure compliance with Medicare and Medicaid standards.
• Arrange necessary resources such as DME, SNF, and transport for patients.
• Document assessments, care plans, and progress notes in the electronic health record system (Epic).

Required Experience / Certifications / Licensure

• Minimum of 2 years of Acute Care Case Management or Discharge Planning experience.
• Must have active California state license; pending licenses will be accepted but must be obtained prior to start date.
• Experience with admission criteria, care coordination, and disease management.
• Knowledge of community resources and agencies related to patient care.
• Experience with the electronic charting system Epic is required.
