Job Opportunity: Registered Nurse

Our organization is looking for a dedicated Registered Nurse to deliver skilled and compassionate in-home care throughout Western Colorado. This position involves a variety of responsibilities aimed at enhancing the well-being of our patients.

• Conduct thorough patient assessments to evaluate health needs.
• Develop and regularly update personalized care plans.
• Administer medications and treatments as prescribed.
• Manage wound care, post-surgical recovery, and chronic disease conditions.
• Work collaboratively with an interdisciplinary team to ensure comprehensive care.
• Educate patients and their families about health management and care processes.
• Accurately document all aspects of patient care.
• Triage patient needs and coordinate necessary services.
• Support safe transitions of care for patients.
• Assist individuals in maintaining their independence and enhancing their quality of life at home.

• Current Registered Nurse license in the state of Colorado.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and interpersonal abilities.
• Experience in home health care is preferred but not mandatory.
• Ability to work collaboratively within a team environment.

This role involves providing care in patients' homes, which may require travel within the Western Colorado region. Flexibility in scheduling may be necessary to accommodate patient needs.
