Requirements

• Active Oregon Occupational Therapist license required
• Minimum 1 year of skilled Home Health experience required
• OASIS experience required
• BCLS certification required
• First-time travelers welcome
• Must have reliable personal transportation
• COVID vaccination series and booster required per facility policy

Schedule

• 8-Hour Day Shifts
• Monday–Friday
• No weekend requirements
• No on-call requirements
• Block scheduling not available

Job Description

We are seeking an experienced Occupational Therapist for a Home Health assignment serving Portland, Oregon, and surrounding communities. The ideal candidate will provide skilled therapy services to patients in their homes, helping them improve functional independence, mobility, safety, and quality of life.

Responsibilities

• Evaluate patients and develop individualized occupational therapy treatment plans
• Provide skilled therapeutic interventions in the home setting
• Assess functional abilities and recommend adaptive equipment as appropriate
• Educate patients and caregivers on treatment plans and safety strategies
• Complete OASIS assessments and documentation accurately and timely
• Collaborate with physicians, nurses, therapists, and interdisciplinary care teams
• Maintain compliance with home health regulations and documentation standards
• Utilize electronic documentation systems for patient records and care planning

Productivity Expectations

• Patient Ratio: 1:1
• Up to 6 patient visits per day
• Standard productivity expectation of 30 points
