Work Environment

This role is set in the unique environment of home care, where therapists provide one-on-one treatment tailored to the individual needs of each patient. You will engage directly with patients in their homes, gaining insight into their everyday challenges and progress. Key Responsibilities

• Conduct thorough evaluations of patients' functional abilities and limitations to determine appropriate treatment plans.
• Develop, implement, and modify customized therapy programs based on each patient's progress and ongoing assessment.
• Deliver a variety of therapeutic interventions, including exercises and manual therapy techniques, while ensuring compliance with safety protocols.
• Educate patients and their families on home exercises, lifestyle modifications, and preventive care strategies to enhance recovery.
• Collaborate with interdisciplinary teams to discuss patient progress and modify care plans as needed.
• Maintain accurate, timely documentation of patient interactions, treatment plans, and outcomes utilizing electronic health record systems.
• Participate actively in staff meetings, clinical supervision, and educational opportunities to enhance professional development.
• Engage in community outreach initiatives and continuous quality improvement (CQI) projects to elevate organizational standards.

Patient Population / Client Focus

The therapist will work primarily with patients recovering from orthopedic surgeries, injuries, and chronic conditions affecting mobility and function. This role requires sensitivity to a diverse patient population, considering various cultural backgrounds and age groups. You will play a critical role in helping patients regain independence and improve their quality of life through effective rehabilitation strategies.

Qualifications

• A Doctorate in Physical Therapy from an accredited institution is required.
• Current physical therapy licensure in the State of New Jersey.
• Strong verbal and written communication skills to effectively interact with patients and healthcare professionals.
• Ability to perform independent evaluations and create age-appropriate treatment plans.
• Demonstrated commitment to ethical practices and adherence to legal regulations pertaining to physical therapy.

Preferred Experience

• Experience in orthopedic physical therapy or home health care is highly desirable.

Compensation and Benefits

The compensation for the Orthopedic Home Care Physical Therapist position ranges from USD 100,000 to 120,000 annually, contingent upon experience and qualifications.
