- Credentialing time frame: 60-90 days from completion of facility application
- Schedule: 4x10 non consecutive or 3x12 non consecutive. 12s preferred ; No Call
- Case Types: General, Endo
- Providers must be able to support: EP labs, Watchman procedures, Spinal anesthesia, Pediatric cases for patients ages 10 and over
- EMR: EPIC
