About the Role:

As a Full-Time Physical Therapist Assistant (PTA) at Walnut Creek Nursing and Rehab, you will play a vital role in helping our residents achieve their healthcare goals. You will assist our physical therapists in creating and implementing individualized plans of care, providing hands-on patient treatment, and collaborating with our multidisciplinary team to ensure the highest level of care. Your expertise and dedication will make a significant difference in the lives of our residents.

Responsibilities:

• Assist physical therapists in the development and implementation of patient treatment plans

• Provide hands-on patient treatment, including exercises, modalities, and other interventions

• Conduct patient assessments and evaluations to identify areas of improvement

• Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination

• Participate in patient education and discharge planning

• Maintain accurate and timely records of patient care and progress

Requirements:

• Licensed Physical Therapist Assistant in Pennsylvania

• Indirect License preferred

• SNF/LTC experience is a plus

• Experience with Net Health is a plus

• Strong communication and teamwork skills
