Overview White House Healthcare Center , a privately owned skilled nursing facility in Orange, NJ is seeking a Part Time Social Worker to join their team of highly skilled professionals. The Center offers strong managerial support and is well known for its outstanding patient care and exemplary customer service. Responsibilities Develop preliminary/comprehensive assessments for each resident. Report any issues or changes to the Director of Social Services. Encourage the resident/family to participate in the developmental process and review of the resident’s plan of care. Develop a written plan of care for every resident that identifies all social problems and needs, followed by setting goals to be accomplished for each problem/need identified. Ensure that all social services components of the MDS reports are completed and signed in the most efficient manner. Review nurse’s documentation to determine if the care plan in place is being followed properly. Job Type Part‑time – 24 hours a week between 9:00 a.m. and 5:00 p.m., Monday through Friday. License/Certification CSW or LSW required. One year of nursing home social worker experience preferred. Experience Social work nursing home: 1 year (Required). Work Location In person. #J-18808-Ljbffr
