Healthcare LCSW

The incumbent in this job provides diagnostic and therapeutic services to individuals with mental or behavioral disorders in collaboration with other health care professionals. In addition, he or she assists patients with various social challenges by providing appropriate referrals to community base organizations, faith based organizations, and hospital programs. The incumbents in this job have routine access to individually identifiable health information including the patients' entire medical records on a need-to-know basis only. All employees of NCHC are responsible to practice good risk management, quality assurance, and deliver excellent internal and external customer service. Prepares and reviews case histories and obtains data through client and family interviews.

Prepares and delivers appropriate treatment plans and psychotherapeutic services in individual or group settings on a client-centered basis, or in tandem with other behavioral health professionals designed to alleviate symptoms of mental or behavioral disorders, psychosocial stress, relationship problems, or difficulties coping with social environment, and to improve overall quality of life.

Coordinates referrals to other health care providers, agency and community based organizations and other entities for best possible care.

Provides social work consultation including but not limited to ongoing case discussion and evaluation focusing on social work data, clinical goals and treatment plans.

Maintains accurate and legible client records in compliance with NCHC policies.

Assist in the development of department policies and procedures.

Submits patient encounters in a timely manner for appropriate billing of patient services.

Participates in health center initiatives and collaboratives that may require supervision of other NCHC personnel in the group's efforts.

Minimum master's degree in social work or clinical psychology. • Licensed by the state of New Jersey as a clinical social worker. • Considerable knowledge of state and federal public health laws specifically in regard to Medicine. As a condition of employment, we require that all staff are fully vaccinated against COVID-19. Therefore, all new employees need to be fully vaccinated against COVID-19 as well as boosters when eligible, and are required to provide documentation of receiving COVID-19 vaccines.
