Key Responsibilities Evaluate and assess patient mental health needs, utilizing the Diagnostic and Statistical Manual of Mental Disorders (DSM-5) to develop tailored treatment plans that respect cultural diversity and engage patients holistically. For instance, you may conduct initial assessments via telehealth and adjust treatment options based on patient feedback and progress. Continuously monitor and adapt patient care plans based on ongoing assessments, employing therapeutic interventions that align with best practices and patient-specific goals. Document treatment records and maintain accurate clinical data in compliance with regulatory standards, ensuring all communications and updates are timely and thorough. Patient Population / Client Focus As a Licensed Clinical Social Worker at Habit Growth Solutions, you will primarily serve a diverse patient population within a community health clinic setting. Patients may present with a range of mental health challenges, including anxiety, depression, trauma, and behavioral issues, necessitating a culturally competent approach that recognizes and respects individuals' unique backgrounds and experiences. Qualifications A Master's degree in Social Work from an accredited institution. Active LCSW licensure in the state of California. Proficiency in utilizing digital tools for teletherapy and electronic health records. A minimum of two (2) years of relevant clinical experience in a mental health setting. Capacity to safely navigate the physical demands of the role, including the ability to lift up to 25 pounds if necessary. Preferred Experience Experience in community health settings, particularly with diverse and under-served populations.
