Located in Muscoda, Wisconsin, United States, Rivers Edge Nursing & Rehab is a leading provider of exceptional care and rehabilitation services. As an integral member of our team, we are seeking a highly skilled and compassionate Occupational Therapist (OT) to join our per diem staff.

About the Role:

As an Occupational Therapist (OT) at Rivers Edge Nursing & Rehab, you will have the opportunity to work with a diverse range of patients, from young adults to seniors, and help them recover from injuries, illnesses, or disabilities. Your expertise will enable individuals to develop, recover, or maintain the skills needed for daily living and work. You will work closely with our interdisciplinary rehabilitation team to create personalized treatment plans that cater to each patient's unique needs and goals.

Responsibilities:

• Conduct thorough evaluations and assessments to identify patient needs and develop effective treatment plans

• Implement and modify occupational therapy interventions to achieve patient goals and improve functional abilities

• Collaborate with patients, families, and healthcare professionals to ensure seamless care transitions and optimal outcomes

• Document patient progress and outcomes in accordance with our electronic medical record

• Stay current with industry developments and best practices to maintain expertise and enhance patient care
