Your Role As a Hospice RN, you'll provide compassionate, patient‑centered care while coordinating with an interdisciplinary team to ensure patients' physical, emotional, and spiritual needs are met. What You’ll Do Plan, coordinate, and deliver hospice care according to individualized patient care plans. Collaborate with patients, families, and the interdisciplinary team (IDG) to ensure seamless care delivery. Conduct comprehensive assessments and develop care plans tailored to each patient's needs. Provide education and guidance to patients and families about the dying process and healthcare decisions. Perform and document patient evaluations, visits, and care in compliance with federal, state, and accreditation standards. Monitor patient conditions, report changes to the attending physician, and adjust care plans accordingly. Supervise and evaluate care provided by Hospice Aides and LPNs, ensuring high‑quality service delivery. Maintain an up‑to‑date understanding of hospice regulations, current drug therapies, and best practices. Offer dietary counseling, addressing changes in diet at the end of life. Participate in IDG meetings, QAPI activities, and staff development opportunities. Qualifications Graduate of an accredited School of Nursing. Current RN license in IL. Minimum of 1 year of RN experience; hospice experience preferred. Exceptional assessment and problem‑solving skills. Strong organizational and time‑management abilities. Effective verbal and written communication skills. Licensed driver with a reliable, insured vehicle. Commitment to maintaining patient dignity, safety, and comfort.
